Forms
Please fill out each of the following forms and bring them with you to your first appointment. This will allow us to begin working right away. If you are a participating member of a health insurance provider, please bring a copy of your insurance card and driver's license with you.
The Orlando Counseling Center for Children, Families and Adults P.A. accepts a variety of insurance plans. It is the responsibility of the client to be certain of their status with their insurance program and to obtain pre-certification for services. Your insurance provider will tell you of your benefits, including the number of sessions that you are covered for, and the amount of your co-pay. This should be confirmed before your first visit.
Co pays are due at the time of service.
